Sign ups & Info
Students interested in joining the Farragut Track and Field Team program should begin by clicking the link below and completing the 2021 sign up form. This will notify the coach of your interest in joining and have your name added to the email distribution list to receive additional information and updates about the program.
Once student athletes are ready to commit to participating in the program, each athlete must submit the required participation forms listed below along with payment for team fees and school insurance no later than February 10, 2020.
Athletes can submit paperwork with payment to the school office, at practice, or during any team meeting. For families wishing to pay team fees using a credit card online, we have added the option to submit payment for team fees and make additional donations using our secured PayPal account.
The following forms are REQUIRED by Knox County to be turned in PRIOR to any preseason or in-season participation.

Clearance Form -or- Physical Exam/History Form

Athletes need only turn in the Physical Exam Form OR the Clearance Form. The Clearance Form is used in lieu of the Physical Exam Form when HIPAA concerns are present.

Please note the Physical Exam/Clearance Form must be dated on/after April 15th of the current school year.

Medical Release Form This form does not need to be notarized.
